| Introduction for practitioners |
The CPHH is a comprehensive history form designed to be administered
or self completed by any client where there is a need to take or review
a history. Clients when initially self reporting can take their own time
and consult documentation and family members if necessary. Once completed,
the form can be reviewed in advance of consultation for any specific areas
which require further discussion. In legal cases, where relevant and important
matters are left out, this can be demonstrated to be an omission of the
client rather than the history taker - in case later this is disputed.
The questionnaire covers a broad range of collateral histories not all
of which are likely to be relevant to a particular client. The CPHH comes
on a floppy disk in Microsoft Word and can be edited in advance
to suit the individual client. Where collateral informants are available,
parts of the CPHH can be administered or self reported by such informants
in respect of the client being assessed.
